Bereavement Coordinator - MSW - Hospice

Role Purpose:
Under general supervision, provides assistance to patients and their families in the form of counselling, resource referral and long term planning. Interviews patients and family members obtaining social history; evaluates insurance coverage; communicates with external payors; makes arrangements for alternative payment plans; counsels and educates patients and families; completes discharge arrangements; and obtains follow-up information. Responsible for the delivery of age appropriate care for assigned neo-natal, child, adolescent, adult and geriatric patients with a wide variety of medical surgical conditions. Under general direction, coordinates the Hospice bereavement program for surviving family members. Counsels and educates patients and families; obtains follow-up information and gathers data. Responsible for the delivery of care for children, adolescents, adults and geriatric family members.

    • Performs patient assessments including: biophysical, psychosocial, spiritual, cultural, and age appropriate needs of the patients, including the assessment for signs/symptoms of abuse or neglect in order to develop, evaluate, and modify patient's plan of care.
    • Interviews patients, appropriate family members and significant others to obtain bio psychosocial history. Provides options for alternative arrangements utilizing external sources or community resources, and may coordinate volunteer and bereavement counselling.
    • Assists physicians and other members of the health care team to identify discharge needs.
    • Provides counselling and education for emotional, social and financial problems of patients and families.
    • Provides direct care to hospice patients, the position also includes bereavement care and coordination and volunteer management work.
    • Reviews and signs off on documentation of initial evaluation and discharge planning performed by the Social Work Assistant. Monitors daily work activities of social work students as required by C.S.W.E.
    • Meets or exceeds established productivity standard while maintaining quality patient care.
    • Initiates and maintains the bereavement program at time intervals specified in the Hospice policies and procedures. Serves as a resource to the team including volunteers.
    • Coordinates bereavement support groups. Assists with Volunteer/psychosocial needs in department as directed by Supervisor/Manager.
    • May performs introductory visits at hospitals, nursing facilities and/or home environments for patients coming onto Hospice service and/or obtains patient or designee's signature on consents (legal documents) as appropriate.
    • May provide comfort and/or supportive visits for defined patients, their families and/or caregivers as is needed. Performs on-call requirements as needed to support branch
    • Performs other duties as assigned.

    Minimum Requirements:

    • Master's Degree required
    • Valid drivers license and a driver's record deemed acceptable by SSM required Missouri MSW license required
    • 1 to 3 years of experience required
Department Specific:

Requires effective oral and written communication skills to communicate with customers. Ability to assess patient/family needs. Requires strong interpersonal skills in order to counsel patients/families, interact with other hospice staff, including volunteers, and outside community agencies in a caring, courteous and professional manner. The ability to assess patient and family needs. Requires data gathering and completion of reports. This position requires daily travel to patient homes and meeting locations, the ability to operate a vehicle in a safe and lawful manner is an essential function of the job. Additionally, access to a vehicle in good working condition required. . Must maintain and be able to produce proof of current auto insurance with at least state required minimum coverage.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
